Appointments:


When calling to make your first appointment, have your insurance cards available to give the insurance information to the intake coordinator. We do our best to schedule appointments on days or times that work best for you. Sessions are anywhere from 45-60 minutes. Your therapist will recommend the frequency of appointments at your first meeting. It is your responsibility to schedule appointments. Bring your ID and insurance card with you to your first appointment. Also bring a list of all your medications. Please arrive 20 minutes prior to your first appointment if you need to fill out new client paperwork.

Cancellations and Missed Appointments:


We ask you notify us 24 hours in advance if you are unable to keep your appointment. This gives us time to make the appointment available to someone else. Each appointment is a special time reserved just for you. It is your responsibility to give us adequate notice. Your appointment will be counted as a “Missed” appointment if you do not give us 24 hours' notice. You may be billed a missed appointment fee of $50. If you have 3 missed appointments within 6 months you may be discharged.


To cancel an appointment you may call us during normal office hours or leave a message on our answering machine after regular business hours and during weekends. We do make reminders calls one business day prior to your scheduled appointment. This is a courtesy call. It is your responsibility to know when your appointments are and to remember them. It is also your responsibility to update us if you have a phone number change.

Insurance:


We will gladly call your insurance company for you to check your outpatient mental health benefits. We will call you prior to your first appointment (time permitting) and provide you with the information your insurance company gave us. We cannot guarantee the information given to you because your insurance company gives us a disclaimer that it could change. We do our best to give you as much information upfront regarding your copay, deductibles, etc. You will be responsible for any copays or deductibles at time of service. You are also responsible for letting us know if your insurance changes. Charges for missed appointments, disability forms, letters, and telephone calls cannot be submitted to your insurance carrier.

Self Pay:


We offer a self-pay option if you do not have insurance, or do not want the appointments to be billed through insurance. This may be due to high deductibles/co-pays. We believe everyone deserves to get help, regardless of income or insurance. Self Pay rates are as follows:


Individual Appointments:

Initial Appointment - $100

Follow-Up Appointments - $75


Couples/Family Appointments:

Initial Appointment - $120

Follow-Up Appointments - $100
